"This patient has cyanosed fingers with also evidence of ischemia (patches of white). There is also atrophy finger pulps.

This could be Raynaud's Phenomenom or Raynaud's Disease.

    Raynaund's Phenomenom has varioud causes
    (1) Autoimmune disease such as scleroderma, RA,

    (2) Occupational-vibrating tools or exposure to cold

    (3) Arterial disease such as thoraci outlet syndrome

    (4) Drugs beta-blockers, ergot alaloid, vincristine

    (5) others-cyroglobulins"

  8. In view of wasting of small muscles of the hand, I would suspect thoracic outlet syndrome / cervical rib as the probable cause.
